Opportunity:
• TJC-accredited acute care hospital
• Oversight of the inpatient Case Management program, including utilization management, care facilitation, and discharge planning
• Direct leadership of Case Managers and Social Workers
• Opportunity to drive performance improvement initiatives related to length of stay, avoidable days, readmissions, and resource utilization
• Serve as a key liaison with physicians on level of care determinations, bed placement, and treatment planning
• Lead daily multidisciplinary rounds to ensure coordinated, patient-centered care
• Play an active role in the Utilization Review and Revenue Cycle committees
• Ensure documentation and workflows meet CMS, Joint Commission, payer, and regulatory requirements
• Guide policy development and process improvement across the care continuum
• Foster a culture of accountability, collaboration, and continuous improvement within the department
• Provide regular reporting and insights to hospital leadership on key case management and utilization metrics
Qualifications:
• BSN preferred
• Active RN (compact)
• Minimum of two years of hospital-based Case Management experience (utilization management, discharge planning, or related cost/quality programs)
• Two to three years of prior management or leadership experience preferred
• Minimum of two years of acute care nursing experience
• Strong working knowledge of Medicare, managed care, inpatient/outpatient services, and the home health continuum
• Demonstrated ability to collaborate effectively with physicians and interdisciplinary teams
• Experience with performance improvement concepts, data trending, and regulatory compliance
• Proficiency with timekeeping and staffing systems (Kronos experience a plus)
