• TJC Accredited Acute Care Hospital
• Oversight of a comprehensive case management program with strong interdisciplinary collaboration
• A chance to drive performance improvement, utilization review, and care coordination across the continuum
• Forge strong partnerships with physicians, nursing leaders, social services, and interdisciplinary teams to ensure seamless care coordination
• Lead the development and refinement of policies that enhance discharge planning, care transitions, and patient flow efficiency
• Champion initiatives to optimize length of stay while safeguarding excellence in patient care and satisfaction
• Serve as the department’s authority on case management protocols, regulatory standards, and industry best practices
• Cultivate a culture of collaboration, accountability, and continuous innovation within a high-functioning team
• Deliver strategic insights through regular reporting and presentations to hospital leadership on key metrics and outcomes
Qualifications:
• A Bachelor’s degree in Nursing is required; a Master’s degree is preferred
• Active RN license in any U.S. state
• Minimum of five years of progressive leadership in case management within an acute care environment
• Deep understanding of utilization review, discharge planning, regulatory compliance, and care coordination across the continuum
• Proven leadership and communication skills, with the ability to influence and align stakeholders across multiple disciplines
• An analytical mindset with the ability to interpret complex data and drive meaningful performance improvement
• Demonstrated success in leading cross-functional initiatives that advance both patient care and operational efficiency
• Proficiency with electronic medical record platforms and clinical data reporting tools
• Strong relationship-building abilities across all levels of hospital staff, fostering collaboration and mutual accountability
