Director of System Care Coordination | RN | Midwest

• Reports to Vice President – System Care Coordination, Quality, & Patient Safety
• Works collaboratively with business unit leadership to ensure care coordination needs of the organization are being met
• Leads and supports Care Coordination Managers and Educators
• Identifies trends in service provision and resource utilization
• Partners with leadership to develop strategic direction for areas of responsibility to help achieve strategic goals
• Assumes responsibility for the organization, analysis, aggregation, validation, and reporting of clinical and financial data/information related to practice patterns, utilization, and/or outcomes
• Works with Care Coordination Managers to establish best practice rounding for outcomes
• Leads and participates in projects designed to enhance service delivery and patient experience
• Coordinates with the Finance department to develop operating and capital budgets for appropriate cost centers
• Facilitates team building, including interpersonal relationships, conflict management, and change management
• Assures compliance with department and organization policies and procedures
• Assists with preparing for The Joint Commission and NCQA when necessary
• Participates in the design of the electronic medical record supporting care coordination, including system updates, build, and ongoing improvement projects


• Bachelor’s degree required, Master’s degree preferred
• RN license (Registered Nurse)
• Minimum of five years of experience in care management or utilization review
• Minimum of three years of leadership experience in care coordination, case management, utilization review, or closely related area
• Nationally recognized certification preferred, i.e., Certified Professional in Healthcare Quality (CPHQ), Certified Case Manager (CCM), Certified Professional Utilization Management (CPUM), Certified Professional Utilization Review (CPUR),
• Knowledge of HEDIS, PQRI, Core Measures, NCQA
• Demonstrated mastery of communication and leadership behavioral competencies, as well as a demonstrated ability to work effectively with interdisciplinary teams
