Galileo Search is conducting a national search for an experienced System Director of Case Management for a nationally recognized and award-winning integrated healthcare system, known for their quality of care, clinical performance and superb ratings for overall customer experience.

The Director of Case Management serves as the system-wide manager responsible for defining the case management model, as well as planning, organizing, staffing, directing, and coordinating the scopes of services for the Case Management and Social Work Services departments across the Health System.

Key Responsibilities and Duties:

•The Regional Director will provide clinical and administrative leadership for all case management and social work services performed
•There are five multi-site Director level FTEs reporting to this position
•Functions as an integral part of the leadership team and works closely with other leaders to strive for continuous improvement
•Provides regional accountability and oversight for the development of structures, processes, and initiatives designed to improve care coordination projects that will define the continuum of care for each patient population and best practice solutions for care coordination
•Responsible for developing a professional, team-oriented, service culture that contributes to the goals of the department and healthcare system to include a plan for education, communication, training and implementation planning
•Serves as a regional leader for oversight of site care management leaders to ensure delivery of the vision, strategies, and road map including utilization outcomes, process metrics, and financial performance of all value-based contracts which have a significant impact on the organization's financial performance
•Designs and implements programs to promote patient advocacy and customer satisfaction with services
•Acts as a liaison with community agencies and programs that interrelate with the hospital systems needs
•Will assist in the management of observation patients


•Bachelor's Degree required or Master’s degree in Social Work
•Registered Nurse (RN) or Social Work license, LISW or LISW-S
•Five years of proven system-level/multi-site leadership/management experience
•Minimum of eight years of experience in the field of clinical nursing care or medical social work in an acute care clinical setting
•Leadership skills and abilities to include human relation skills, crisis intervention, critical thinking and decision making, conflict management
•Experience with payer and regulatory rules
•Experience implementing a system-wide standardization operational model
•Ability to generate and support new ideas and innovations, while implementing changes in the Case Management process
•Ability to apply experience, analysis, and intuition to maintain and improve the quality of services provided by the Case Management and Social Work Services departments
•Ability to communicate and integrate with the business/financial, nursing and interdisciplinary teams
•Possesses knowledge of legal, regulatory, and accrediting standards related to the utilization management and discharge planning
